Need a Medical ID Card?

You can now download and print your Medical ID Card directly from the PEAK website: www.colorado.gov/PEAK.

Lost or need a new EBT card?

To request a replacement card or change your PIN, call 1-888-328-2656. You can also report a lost, stolen, or damaged card.
