Your client will get connected with a Resource Navigator. They will help your client find resources like food assistance, childcare, and behavioral health.

Nurse-Family Partnership

(NFP)

For first-time parents. Specially trained Registered Nurses help with topics including:  

• Prenatal and postpartum education.

• Breastfeeding.

• Birth control.

• Parenting skills.

Primary parent’s income determines eligibility.  

Refer to NFP from early in pregnancy through 30 days after delivery.

Supplemental Program for Women, Infants, & Children

(WIC)

For pregnant, postpartum people, and families with children under age 5.  

Trained WIC Educators and Registered Dieticians help with:

• Nutrition education and financial help with food.

• Baby feeding support.

• Healthy weight monitoring.

• Screen for anemia.

Families' income determines eligibility.  

Refer to WIC from pregnancy through children up to age 5.

Public Health Clinic

Anyone, including those with Medicaid, not enough insurance or no insurance, or private insurance.

Certified Nurse Midwives & Registered Nurses help with:  

• Adult, child, and travel immunizations.

• Birth control.

• STI screenings.

• Pregnancy tests.

Sliding fee or discounts for qualified services.

HCP

Children from birth to age 21 with special health care needs:  

• Behavioral.

• Physical.

• Developmental.

• Emotional.

Care Coordinators help with:

• Local services & supports.

• Appointment scheduling.

• Provider communication.

• Community resources.

Free for children with special health care needs, regardless of income or diagnosis.
