Mesa County Public Health releases a Community Health Needs Assessment (CHNA) every three years in partnership with non-profit hospitals and organizations. This facilitates a more unified approach to improving health and quality of life in our community.
This approach is rooted in the belief that community-driven solutions are more effective.

Updated Community Health Needs Assessment (7/11/2024)
The 7/11/2024 version corrects a chart title on page 177. The chart reflects the count of overdose deaths, not the rate per 100,000 people.
New tool for community partners accessing CHNA data
Community Health Improvement Plan
A Community Health Improvement Plan (CHIP) is part of the Community Health Assessment and Improvement cycle.
The CHIP follows the CHNA and identifies specific priorities for action and progress over the next few years.
