Eligibility for Paratransit

To qualify for paratransit service, a Paratransit Eligibility Application must be completed. This includes the Paratransit Applicant Questionnaire and the Professional Verification Form

Both the Paratransit Applicant Questionnaire and the Professional Verification Form must be submitted before the application process can move forward.

Please allow 3 weeks for Grand Valley Transit (GVT) to process your paratransit applications. Applications will not be deemed complete until the professional verification is received and the customer interview is scheduled. Incomplete applications will not be processed.
